Housing Stabilization Services

Housing Stabilization Services (HSS) is a new benefit for individuals on Medical Assistance to help find and maintain housing. Support to help individuals with disabilities, disabling conditions, seniors, and their families in finding and maintaining housing. Three types of services: housing transition, sustaining, and consultation services

Housing Consultation

Assists eligible individuals who do not have access to a case manager or targeted case manager in developing a person-centered plan and submitting it to a Housing Transition provider. Services that assist a person in developing a housing-focused, person-centered plan, obtaining the right state plan resources that support housing stability, and providing referrals or information about other services. People on Medical Assistance who do not have a waiver case manager, mental health targeted case manager, or MSHO/MSC+ care coordinator to conduct an assessment and a person-centered plan of treatment can use this service.
